Nomination Form - Missouri Inline Hockey Association

MOIHA Hall of Fame Nomination Procedures:

All nominations must be by completion of the official MOIHA Hall of Fame Nomination Form (below). Nominations must be received by April 15th of the year to be considered.
Nomination for any individual is in one category only — Player, Team, Builder or Referee. A nomination for an individual in more than one category requires specific approval by the MOIHA Board of Directors.
To be eligible for a "Player" or "Team" nomination, a candidate must have been actively involved with MOIHA for a minimum of 1 year.
To be eligible for a "Builder" or "Referee" nomination, a candidate must have been actively involved or officiated in MOIHA for a minimum of 4 years. A “Builder” candidate is for any person, in any phase of the sport, such as organizer, coach, manager, supporter, etc.
